The following forms must be filled out and returned in the postage paid envelope that you received in the mail. This year, all new student athletes are required to have a physical performed by a family doctor prior to the beginning of practice.  A copy of the Olivet College Pre-Participation exam is available below.

Complete the required information (listed above) and return them by August 1, 2010 to the Olivet College training room or on the day you check-in, please bring them to the training room.
