          Athletic Pre-Participation Health History Form (AT FORM #2)

We also ask that you also read through the following information prior to completing the forms listed above. These letters and documents provide further explanation and information regarding athletic participation at Olivet College.

Complete the required information (listed above) and return them by August 1, 2010 to the Olivet College training room or on the day you check-in, please bring them to the training room.
